Registration Cancellation Form

  • Please select the program you are registered for. If other, enter the program below.
  • Parent’s Full Name for Child Registrant
  • Please indicate which month you would like to cancel your registration. If your request is received by the 15th of the month, your membership can be cancelled for the next month. If it is received after the 15th of the month, your registration will be cancelled the following month. For example, if your request is received on or before June 15, your membership can be cancelled on July 1. If your request is received on or after June 16, your membership will be cancelled on August 1.

Policy for recurring payments

All payments are non-refundable. All payments are billed automatically on the first day of the month/quarter for the entire month/quarter. There are no pro-rated refunds for mid-month/quarter cancellations. Cancellation requests must be submitted by the 15th of current month to terminate recurring payment/membership for the following billing cycle.
